Research Questionnaires
Dr Amaranath is committed to clinical research in his surgical practice. Patient contribution to research forms the foundation for this endeavour.
Patients are encouraged to fill out the relevant research questionnaire below, prior to or on the day of their appointment with Dr Amaranath (the type of questionnaire to be completed will be discussed with you by a member of the team). The answers that are collected from this questionnaire will be used as clinical data, in a confidential manner for the purposes of research and education.
